Align Myotherapy is committed to protecting the privacy of clients within our clinic. Information collected is kept strictly confidential and used only for the purpose of providing care to our patients. Our clinic is dedicated to ensuring clients who receive care from our myotherapist are comfortable in entrusting their health information to this clinic and understand and acknowledge our practice policies. We strive to always ensure that visitors to Align Myotherapy feel that care provided is always in a safe and comfortable environment.
Our Commitment to You
We are committed to working with our clients to ensure that ALL aspects of assessment and therapy are conducted with our clients’ informed consent. Our practitioner will provide clients with comprehensive information about risks and benefits associated with any proposed therapy and/or recommendations, as well as encourage clients to raise queries or concerns regarding any proposed therapies or recommendations. Clients will be provided information about any known alternatives to proposed recommendations and therapy to ensure an informed decision can be made about their care.
Privacy Policy
In accordance with the Privacy Act 1988, Align Myotherapy requires your consent to collect personal and medical information about you.
What Personal Information is collected?
Personal Information is information or an opinion that identifies an individual. Personal Information that we collect include your:
• Full name
• Address
• Contact number
• Email address
We may need to use this information for the following purposes:
• Administration within our clinic, including billing
• Contact with other health practitioners involved in your health care using names, addresses, and phone numbers you have given us.
• To provide quality health care.
Collecting information
When we collect personal information from an individual, we will ensure we do so in a fair and honest manner and that we disclose how this information will be used other than those for which we have obtained the individual’s consent, unless the law requires otherwise, or other exceptional circumstances prevail as described under the Act. You may, at any time, contact our clinic, regarding your personal information. If we collect sensitive information (as defined under the Act), we will treat it with the utmost security and confidentiality. We will ensure that it is not collected for any purposes other than your direct care.
All medical and personal information we collect are held with the strictest confidence.
No release of information will take place without prior consent from the patient.

Late/Cancellation Policy
Your appointments and wellbeing are very important to us. We understand that sometimes unexpected events can occur, causing schedule adjustments. If you need to cancel your appointment, we respectfully request at least 24-hours notice.
Our Policy:
• Any cancellation made less than 24 hours prior to your scheduled appointment will result in a cancellation fee. The amount of the fee will be equal to 50% of the consultation fee. No-shows who provide no notice of cancellation will incur a fee equal to 100% of the consulation fee.
• Any reschedule made less than 24 hours will be given a once-off $0 cancellation fee in the first instance, per client. Any subsequent reschedules will be treated as a cancellation and will incur a fee equal to 50% of the consulation fee.
• If you are more than 15 minutes late for your appointment, we may be unable to accommodate you. In this case, the same cancellation fee will apply. We will do our very best to reschedule your appointment for another time that is convenient to you.
OH&S Policy
Align Myotherapy is dedicated to the emotional and physical safety of all staff, clients and visitors, and, as such, any acts of violence and/or aggression are completely unacceptable.
The follow are examples of behaviour that will not be tolerated:
• Obscene, racist, sexist or demeaning language
• Aggressive behaviour, harassment and intimidation
• Rude or threatening language or name calling
Any members behaving in a way that does not comply with our clinic policy will be asked to remove themselves from the premises. Violent behaviours may be reported to the appropriate authorities if deemed necessary to protect our staff and clientele.
